Parkway Cardiology Associates participates in most health plans. Our list is continuously being updated. Please call our office for current information or questions regarding your coverage.

Fees, Payment and Insurance
Our fees are based on reasonable and customary charges for this area. The fee for the first visit is higher than a routine follow-up visit because of the amount of time required to evaluate the patient, obtain a thorough medical history and process the paperwork.
We have a charge to cover clerical expenses for completing disability forms and Attending Physician Statements if you apply for new insurance coverage. We request that these forms be submitted as early as possible. We often are not able to complete these forms while you wait. We will request that you leave the forms, and they will be mailed to your home upon completion.

Please be prepared to pay for your medical charges at the time of your office visit. We will supply you with billing material so that you can bill your insurance company. For expensive testing done in the office and all hospital charges, we will file most plans. Please ask if we participate with your insurance plan. We will file insurance for Medicare and Medicaid of Tennessee. We will be happy to help you with any questions you may have concerning your insurance so that you may receive maximum benefits. Your insurance plan may require referrals from your primary care physician in order for you to see a specialist. Although we will be glad to assist you, it is your responsibility to comply with your insurance requirements. If you have any questions, concerns, or if some financial hardship exists, please feel free to speak to our office staff.

Past Due Accounts
Although we request timely payment for our services, we encourage you to discuss with our staff any problems that are anticipated with payment. Time will be extended for payment that is fair to you and to your doctor. If you do not make such arrangements, your account is considered past due 90 days after you receive service.
